Forms must be completed and brought to the SML dock on the day of departure. Group leaders may elect to collect forms from participants before the day of the trip. Participants will not be permitted to board SML vessels without signed release forms.

Day Trip Forms
For Minors (under 18), click to view/print packet.
- Parental Consent Release Form (Indemnification) – must be signed by a parent or legal guardian
- Multimedia Release and Agreement Form – please alert SML staff if a parent/guardian does not wish to give SML permission to use photo/video of his/her child in SML media.
